Provider Scheduling Information
Using the Online Order Form:
- Click HERE to open the online order entry screen in a new window
- Fill out all information the has an asterisk*
- After the necessary information is entered,
click “Submit”
- Once you click “submit,” no additional
editing can be done. If you realize you made
a mistake, you can resubmit a new order.
- Our goal is to reach the patient as soon as
possible. If we cannot reach them within
three (3) business days, our scheduling
department will notify you.
- When we reach the patient, you will receive
a reply with the patient’s appointment
information.
We must have your email address or phone number to notify you of the confirmation.
